Team Sports (3 on 3 Basketball, Bocce and Volleyball only)

Team captain/manager must submit the following items by the entry deadline of January 1, 2012—

• Completed Team Entry and Waiver Form (one completed form per team)
• Waiver Form must be signed by all team members. Please ask all team members to read the Agreement, Release, and Waiver of Liability before signing.
• Total payment = team entry fee + additional item purchases (pins, t-shirts)

Make checks payable to “Senior Olympic Festival.”

Final registration deadline is December 31, 2011 (must be postmarked by). Note: Any registration received after January 1, 2012 will be subject to a $3 per event late fee.

No registrations will be accepted after January 11, 2012 (or postmarked by).

Note: Team/Doubles Registration cannot be done over the phone through EZEE-REG.

Team Entry Fees:

• Basketball 3 0n 3, $60
• Bocce, $24
• Volleyball, $150

All Team Fees must be sent in together!

Team captain/manager should keep a copy of the completed Team Entry and Waiver Form for his/her files.

Changes to rosters may be made. Report any roster changes to the Senior Olympic office as soon as possible.

All participants are to participate in the age category of their 2012 birthday. For example: if a lady is turning 60 in December of 2012, she is to compete in the 60-64 category (even though she will be 59 at the time of competition). *Players must present valid picture I.D. before start of competition.

Refer to the Schedule of Events and Map for dates, times and locations of events to avoid scheduling conflicts and to allow transportation time between events.

Doubles Sports (Badminton, Bridge, Croquet, Pickleball, Pinochle, Shuffleboard, Table Tennis, & Tennis only)

Doubles captain/manager must submit the following items by the entry deadline of January 1, 2012—

• Completed Doubles Entry and Waiver Form (one completed form per doubles partnership)
• Waiver Form must be signed by both doubles members. Please ask doubles partner to read the Agreement, Release, and Waiver of Liability before signing.
• Total payment = doubles entry fee + additional item purchases (pins, t-shirts)
